Lee died on Friday, in a tragic accident on board an offshore platform operated by a company I am working with just now. The story has now been reported in the news, but technical accident details will be released later in the week. Although, I can say, it was a tragically simple oversight of a previously unidentified hazard.
A few people here knew Lee, who was just 32 years old, and by all accounts he was a good man. He is survived by his wife and four children.

We’ve all had close calls offshore. Those of you in positions of leadership and management in safety critical industries know this already, but let’s take a moment to look at our work spaces with fresh eyes, in honour of Lee.
